The QIST is helping practices to redesign their care pathways for Diabetes to increase the percentage of patients who meet the National Diabetes Audit’s ‘3 Treatment Targets’ (HbA1C, Cholesterol, BP).

We are working with a number of practices thinking with them how they improve effectiveness and reduce inefficiency in diabetes care. Our approach is supports and complements the East Haringey CHIN work which is piloting a new model of care for diabetic patients.

We will soon be sending every practice a quarterly report including a list of patients who might require an intervention.
